Pediatric Sedation

There are a number of different medications used for sedation in the pediatric population. A recent study published in Pediatric Emergency Care (Pediatr Emerg Care 2009 Oct; 25:648) suggests that Methohexital is superior to Pentobarbital for children during head CT.

Listed below are the dosings recommended in the study:

Methohexital 1.0 mg/kg – initial IV dose (If sedation is inadequate after the first dose, subsequent doses of 0.5 mg/kg (up to 2.0 mg/kg) given at 5-minute intervals)

Pentobarbital 2.0 mg/kg – initial IV dose (If sedation is inadequate after the first dose, subsequent doses of 2.0 mg/kg of pentobarbital (up to 6.0 mg/kg) given at 5-minute intervals)

If you are performing a painful procedure such as an LP or Laceration Repair, Ketamine is recommended:

Ketamine 1.0 mg/kg

As with every sedation, be sure to monitor oxygen saturation as well as EtCO2.
